  • Geographical area of the services
  • Estimated commencement date of the provision of services

Finland

General authorization

  • Contact information for the operator, including postal address, telephone number, and email address
  • The operator’s business identity code
  • A short account of the company’s business group relations (corporate structure)
  • The geographical area in which the operator operates
  • Estimated date of the commencement of operations

Germany

Open entry

  • Information required to identify the operator or services, including the company register number and its address
  • A brief description of the network or service being provided
  • The date on which the provision of the services or operation of the network is due to begin

Iceland

General authorization

  • Contact information for the service provider
  • The name and contact information of a designated contact person within the service provider’s organization
  • A description of the operations of the service provider
  • A description of the telecommunications infrastructure of the service provider
  • A listing of shareholders that hold a five percent or more interest in the service provider

Ireland

General authorization

  • The name and address of the applicant, including the applicant’s legal name and trade name(s)
  • Company Registration Number, if applicable
  • Name and contact details for the primary/regulatory contact person
  • Name and contact details for an alternate contact person (optional)
  • A description of the network and/or services being offered
  • The geographic region in which the network will be operated or where the services will be provided
  • The estimated date of the commencement of the operation of the network or the provision of the services
  • Identification of connected (or related) companies that are also engaged in the operation of a network or the provision of a service

Malaysia

General authorization

  • Name of the applicant
  • Identification of the legal status of the applicant (e.g., corporation, partnership, individual, society)
  • Address and contact details for the applicant
  • A general description of the facilities/services/activities that will be owned or operated or provided by the service provider
  • The location of the facilities or services

Pakistan

General authorization – Data Class Value Added Service Licence; Voice Class Value Added Service Licence; and Class Value Added Registered Service

  • Name of the service provider, along with its postal address and its registered office address
  • Certificate of incorporation
  • Name of the designated contact person, along with the contact information for this person and his/her Computerized National Identity Card Number
  • Identification of the type of licence or registration (Data Class Value Added Service Licence; Voice Class Value Added Service Licence; or Class Value Added Registered Service)
  • Province in which the services will be provided
  • Identification of the service provider as a commercial or a “not for profit” entity
  • An undertaking that: neither the applicant company nor its directors have ever been declared insolvent by a court of law; neither the directors nor the applicant company have ever been convicted by a court of law for major offences or unethical/immoral turpitude; and that neither the applicant company, its shareholders, or its directors have defaulters of the Pakistani ICT regulator
  • Bio-data of key management personnel
  • A brief description of the applicant company’s business plan

Saudi Arabia

General Authorization – Type B Class Licence (unlimited number)

  • Name and address of applicant service provider
  • Name and contact information of a representative of the applicant
  • Basic service information (identification of the type of licence desired and a description of services to be provided)
  • Bank guarantee
  • Approved copy of the authorized signatory official Ahwal (ID) card
  • Approved valid copy of the Commercial Registration Certificate
  • If the application is for a VSAT Licence, then additional information related to an application for a frequency assignment must also be filed

St. Lucia

General Authorization – Class Licence, Type A (Providers of Telecommunications Services)

  • Identification of the applicant, including contact information, an address for the service of documents, details about its legal structure and its directors, information about the applicant’s corporate registration, and information about its previous involvement in the telecommunications sector
  • Certified copies of the Certificate and Memorandum on Incorporation
  • Identification of a designated contact person and that person’s contact details
  • Identification of services to be provided under the Licence
  • A business and financial plan, including information about the management structure of the applicant, its marketing plan, its operations plan, and its financial plan
  • Audited Financial Statements for the previous three years (if the applicant has been a going concern) or a certified statement of financial affairs of each director (if a new entity)
  • Information about the corporate structure of the applicant, including information about the percentage of shares held in each entity, whether any affiliates have or will apply for a telecommunications licence, the identification of any shareholder holding ten percent or more of the issued and outstanding shares of the applicant, and the identification of any subsidiary that in which the applicant holds at least five percent of the issued and outstanding shares and which is a licensed telecommunications services provider in the OECS
  • Information about the managerial staff of the applicant
  • Information about the applicant’s current business activities in the telecommunications sector, including information about the infrastructure the applicant has in place, details about radio frequency authorizations held by the applicant, information about the products and services that the applicant currently provides, and copies of all current contracts with all classes of customers
  • Information about the applicant’s proposed business activities under the licence, including information about the infrastructure that the applicant plans to put into place and radio frequency authorization applications made or planned to made, a detailed description of all services that the applicant plans to provide (e.g., technical aspects of providing the service, target customer base, and planned access arrangements), and  details of any proposed supply arrangements related to the proposed network or services

Sweden
